Release of Liability and Assumption of Risk | Tryout Release and Waiver of Liability

ORLANDO, Fla. (UCFAthletics.com) - UCF students interested in trying out for the men's soccer team will have the opportunity next week when the program holds walk-on tryouts. A mandatory informational meeting will be held on Monday, Jan. 7, at 12 p.m., inside The Venue at UCF.

A follow-up meeting will be held on Wednesday, Jan. 9, at 12 p.m. At that meeting necessary paperwork must be handed in in order to participate in tryouts on Thursday, Jan. 10, and Friday, Jan. 11, at 9:30 a.m.

Players must provide the following before they will be allowed to try out. (necessary documents in PDF format on the top of this page):

- Physical within the last 6 months
- Proof of sickle cell trait test results (those born in the state of Florida should have results on their birth records)
- Proof of primary insurance - must provide copy of insurance card
- Completion of the forms included at the top of this page - liability and assumption of risk and tryout release and waiver
- Must provide proof that they are enrolled in a full-time program of studies at UCF
